If you are a NYS EPIC (NYS RX drug help program) member you may receive a renewal letter from EPIC within the next few months. (not all EPIC member will receive this). 

This renewal will ask for your previous year income. You are required to complete this form and mail to EPIC within 15 days. If you do not response your EPIC program will cancel on December 31, 2018.

If you are not a member and you may qualify, attached is the EPIC application (complete and mail to Albany)
